Under the administrative supervision of the Health Manager, the Community Health Nurse (CHN) provides a community health program to the Tl’azt’en community. The CHN will provide support and direction to the other program staff on health-related issues.
Reports to: Health Manager and/or nurse

- Knowledge and understanding of First Nations communities, health, and social issues
- Knowledge and understanding of the population health approach and the social detriments of health
- Excellent written and interpersonal communication skills and the ability to work effectively with a variety of people and circumstances
- Ability to teach and facilitate learning
- Ability to work on a team with other nurses and health staff
- Self-directed with demonstrated organizational skills
- Willingness to take training in Trauma-Informed Care and Indigenous Cultural Safety Training, as well as on-going education as needed
- Awareness of professional Scope of Practice and ability to work within it and inform others of abilities and limitations when needed
- Ability to maintain high level of confidentiality and professionalism
- Humility, gentleness, and patience are desirable assets
Job Results and Responsibilities:
- Work as an integral part of the Health Centre team
- Data steward/Owner as per EMR Policy
- Assist in Developing, and implementing Panorama data system for the Community Health Program
- Develop, and implement and evaluate Community Health Programs to better meet the needs of the community
- Counselling parents in pre and post natal periods as needed, including postpartum home visits
- Provide basic life support for life or limb emergencies until emergency services can attend during operational hours only
- Other activities related to the health of the community members as requested by the Health Manager
Position Requirements:
- Degree in Nursing from an approved Canadian College/University or equivalent
- Knowledge/Experience in Community Health nursing an asset
- Registered in good standing with the British Columbia College or Nurses and Midwives (BCCNM)
- Hold a valid Class 5 BC Driver’s License and own vehicle
- Willing to learn about the language and culture of the Carrier people
- Completed criminal record check including vulnerable sector screening
- Current certification in Basic Life Support for Health Care Professionals Performance Evaluation: Annual review
